    An established index is not a requirement for joining BSGT although until one is established you cannot compete for prizes in any tournament. To establish an index you must submit between 7-20 valid scorecards.

I hereby make application for membership in Blue Sky Golf Tours for the 2009 Tour. I understand that I am not eligible for competition until an index has been established for me by the SCGA or WCGC. All members will receive access to the club's By-Laws, tournament schedule, monthly newsletters and all benefits stated herein. I understand that I must abide by the club's By-laws and handicap procedures of the USGA.
